Pickering council's executive committee endorsed the agreement this past Monday and council is ex- pected to approve it this coming Monday. Ajax council has already given its approval. The agreement is expected to be implemented early in the new year. The Automatic Aid Program is designed to allow crews from the closest fire station to respond to an emergency, regardless of municipal boundaries. Though Dr. Donald MacDiarmid, 37, pleaded guilty to one count of fraud over $5,000. Durham Region Crown Attor- ney John Scott told an Oshawa court the general practitioner committed numer- ous breaches of trust. "There's been multiple breaches at all levels... He breached the trust of his pa- tients, fellow doctors, the taxpayers of Ontario and OHIP... and he breached his Hippocratic oath," Mr. Scott said Thurs- day, calling for an "upper -range refor- matory" sentence of less than two years. Court was told Dr. MacDiarmid, of Toronto, defrauded OHIP of $155,000 between January 1998 and January 1999 and the Westney Heights Medical Centra; of approximately the same amount be- tween 1994 and 1997. Detective Catherine O'Connor, of the Ontario Provincial Police's anti -rackets branch, told Judge Donald Halikowski of the Ontario Court of Justice that the OHIP fraud came to light after a manag- er at the medical clinic began noticing discrepancies in the account numbers on Dr. MacDiarmid's claim input remit- tancc forms. The manager alerted the Ministry of Health, which launched its own investi- gation and called in the OPP's Health In- vestigation Unit to begin a criminal probe. Det. O'Connor said as part of stan- dard billing procedure physicians at the clinic would file a claim input form fol- lowing each patient visit, listing the pa- tient's health card number, a diagnostic code and the fee payment code recog- nized by the Ministry of Health. The information would be transferred onto a computer disc and submitted by the clinic to the ministry, which would deposit payments directly to doctors' bank accounts. Court heard the investigation re- vealed the physician had been submit- ting manual claims, bypassing the clinic, to bill OHIP for thousands of bogus pa- tient visits. The claims showed the doc- tor billed OHIP for legitimate patient visits by submitting them to the clinic, and billed for bogus visits on a manual claim card. " 77he patients did not attend on the days that were submitted on the manual cards;' Det. O'Connor testified. Dr. MacDiarmid, who co-operated with police during the investigation "pointed out the extent of the fraud" dur- ing the execution of a search warrant, Det. O'Connor said. Under a verbal agreement, court heard, the clinic was to receive 40 per cent of Dr. MacDiarmid's earnings, to cover overhead costs, but was cheated out of $155,000. During sentencing submissions, de- fence lawyer David Porter said Dr. Mac- Diarmid has lived an "exemplary life" until now and his crimes were out of character. Psychiatrist Dr. Daniel Silver, testify- ing on behalf of the defence, told the court that in his opinion Dr. MacDi- armid's conduct was an indirect result of "inner turmoil" caused by repressed "rage" towards his father. He said the doctor is unlikely to reof- fend, requires extensive psychotherapy and should not be jailed. Mr. Porter said Dr. MacDiarmid is extremely remorseful, has suffered pub- lic humiliation and remains in an "emo- tionally fragile" state. He asked the judge to impose a conditional sentence, to be served at home. The sentencing was adjourned to Jan. 14. Inquest called into three deaths BY CINDY UPSHAIL Staff Writer An inquest into the apparent suicide deaths of three Whitby Mental Health Centre patients will be held in the new year. Dr. Peter Clark, regional coroner for central Ontario, announced Wednesday the decision to hold an inquest into the 1996 deaths of Gregory Timmins, 20, Denise Charles, 24, and Wilhemine Dietl, 49. All three were suffering from :schizophrenia. Mr. Timmins died on May 14, 1996 as a result of injuries sustained after falling from the Brock Street overpass onto Hwy. 401 and Ms. Charles died on June 26, 1996 after falling from the same overpass. Ms. Dietl died on Aug. 6 when she stepped in front of a subway. .Although calling an inquest into the deaths is not mandatory Dr. Clark says it was determined one should be called be- cause of the circumstances. "Within a relatively short period of time you have three individuals who suf- fered from a very serious illness leave 'the centre and die," he said in a inter- view. In addition to these factors "one of the families involved has requested the " inquest be held;' Dr. Clark says. The inquest will study the circum- stances surrounding the deaths, why they happened and will make a determi- nation on the cause of death. It will "try to find out if there is any way any future deaths can be prevented;' he says. Ron Ballantyne, administrator of the Whitby Mental Health Centre, said he has not been told officially an inquest will take place, but says the facility will be part of the proceedings and assist the inquest in any way it can. In the last 12 years the facility has been involved in one other inquest in- volving the death of a patient, which Mr. Ballantyne says is a good record for a fa- cility like the centre where staff are "dealing with people who wish to harm themselves." "We've been responding together for calls on the 401 between the municipalities, but we wanted to have for- mal legislative authority for a broader automatic aid pro- gram;' says Ajax Fire Chief Randy Wilson. A Pickering staff report on the agreement notes it will help ensure the provi- sion of vital fire protection services in the two towns and improve the level of public safety for residents and busi- nesses. Objectives of the new system include: - ensuring the closest fire station responds immediate- ly to an emergency; - immediately assembling an adequate fire attack team, which may be made up of personnel and equipment from either or both fire de- partments; • providing emergency equipment and fire person- nel, particularly at the outer boundaries of each munici- pality, that might not other- wise be available. The report outlines the six fire response situations cov- ered by the agreement. Those include different com- bined response scenarios for three sections of Hwy. 401; the portion of north Picker- ing bounded by Sideline 16 to the west, Hwy. 7 to the north, Lakeridge Road to the east and the Fifth Conces- sion to the south; three sec- tors on the boundary be- tween the two towns; and the Ajax and Pickering Health Centre. The agreement between the two towns follows about a year of discussions on the issue. Among those who welcome the new arrange- ment is Pickering Deputy Fire Chief Bill Douglas. "It's certainly a positive thing;' says Deputy Chief Douglas. 'The bottom line is the safety of the residents and it just makes sense" He explains the two de- partments have been helping each other out for a long time, but a formalized agree- ment should make fire re- sponse times even faster. Police searched a house in the townhouse complex for about 30 minutes sifting for evidence after a suspect was taken into custody. Sev- eral bags of material and a silver piece of metal with a blade at one end were seized. A 21 -year-old man was also in the home police were targeting, but doesn't face any charges, Det. Townley says. A 17 -year-old Ajax youth faces 14 counts of mischief in connection with the case. The youth, who can't be named under provisions of the Young Offenders Act, was remanded in custody for a bail hearing on Monday. They can't agree on what to ask from Santa. Instead, it will be their very own millennium project: come up with a downsizing plan acceptable to the Ministry of Municipal Affairs. Under direction from Minister Tony Clement, who gave Toronto about two days to cut its size from 58 councillors and then agreed on more time. Durham and other regions are having their feet put to the fire. Get smaller on your own or the Province will do it for you. With that in mind. councillors kicked around some of their own ideas this past week. The starting point is 28 councillors from the eight municipali- ties along with the regional chairman. It's too late to change it all now for the November 2tXln election but. make no mistake, there will not be 28 council- lors and a chairman after the 2003 election. Three proposals were brought up Wednesday. 'A'hitbv councillors Joe Drumm and Pat Perkins suggested 15 councillors for the region. These coun- cillors would NOT sit on local council but would represent their municipali- ties at the regional level only. Mayors Diane Hamre of Clarington and Uxbridge's Gerri Lynn O'Connor also suggested 15 councillors — all eight mayors plus seven other councillors elected from the eight municipalities based on population size. And Oshawa Councillor Brian Nicholson and Clar- ington colleague John Mutton would like to see a 20 -member council with only the eight mayors removed. The last suggestion simply doesn't downsize enough and should be reject- ed. It's possible that even 15 is too many. How about a council made up only of the eight mayors and a chair- man elected by the population of the entire region'? The mayors would re- port back to their local councils at each local meeting and take concerns raised by councillors to the Region when they attend meetings there. In addition, mayors are most truly representative of their constituents as they are the only municipal politicians elected by all voters in their municipalities. The mayoralty is the most high-profile, most prestigious local political posi- tion in each municipality and an elec- tion where the mayor would fill a dual role might draw more voter interest and participation. %layors who couldn't make a re- gional meeting could be represented by the municipality's deputy mayor. Mayors would serve on all regional committees or simply fold committee work into their regional schedule. Finally, a nine -member regional council, down from 28, would surely draw approval from the ministry and might save the two-tier level of gov- ernment. If anything can. I find it ironic that the village which shares the town of Picker- ing 's name is not part of that city. That village, of course, is Pickering Village. Without that village, the new City of Pickering will continue as the Town of Pickering has for a good many years — a so-called community without a downtown, without a town core, without a cul- tural or historical centre. As a resident of Brougham, I am regularly asked why my children are "allowed" to play on town dren know nothing about him. Last year at this time our family was on pins and needles waiting for the word on our mother's cancer. We all knew - you just know when it's going to be bad news - and it was the worst. Mom's cancer was the kind you don't beat, it beats you. Over the past 12 months we grew as a family, as we knew we would grow. We dealt with emotions, most of them new to us, head on. We faced the facts, we braced for the end while embracing the present. It was never easy to say goodbye after each visit with mom and dad, not knowing if there would be an- other meeting. It was the arrival of Jackson, and the twins Emily and Stuart which kept mom going for the last few months. It was also her religious belief which kept her spirits high. There were never any complaints, only a quiet strength which saw her through. Her nature wouldn't let her gripe about her plight. I didn't want to write this column but I knew I had to. It's a way to put this whole ordeal on a shelf, some- where hopefully not to be revisited any time soon. Granted, it is an experience my dad and siblings will draw on as the years march forward and we deal with more sorrow. Sorrow, that at this time is still as fresh as the day she left us to be with the angels. The day she left to take over as the angel for six lit- tle grandchildren. Somehow she's more than that though. She was the glue in our family, while our dad was the mortar. Now dad is both by default, but he'll have some help with our family angel on his wing. In one week we'll sit down to Christmas dinner as a changed family. Dressup' all dnewrole COUNCIL BRIEFS Pickering's Ernie Coombs treads boards in `quite different' show ITEC. 19, 1999 BY LINDA WHITE Staff Writer PICKERING — It's been several years since Ernie Coombs retired as TV's 'Mr. Dres- sup', but the Pickering resident still loves to dress up. This time, however, he's dressing up for his role as the father of 'Cinderella' in what pro- ducers dub a "slightly -fractured version of the classic fairy talc", which is playing at Toron- to's Elgin Theatre until Jan. 2. Certainly, performing in a musical stage production is "quite different" from the 30 -or -so years he spent on TV, admits Mr. Coombs, who recently spoke to the News Advertiser on his 72nd birth- day. "Here, I'm just one of many peo- ple involved in the show. I'm stand- ing in the wings" waiting to per- form. And while the Order of Canada recipient talks enthusiastically about performing on stage, it wasn't some- thing he set out to do after retiring from 'Mr. Dressup' at the end of its 1995-96 season. However, after attending a perfor- mance of 'Jack and the Beanstalk' with his daughter and grandchildren shortly - after his retirement, Mr. Coombs recalls being "blown away" by the experience. "It reminded me of thechildren's theatre I had done;' says Mr. Coombs, national spokesman for Save the Chil- dren/Canada. "I told my daughter I would love to be in a show like that:' About three months later, he re- ceived a phone call "out of the blue" from Ross Petty of Ross Petty Pro- ductions, asking if he'd be interest- ed in performing in 'Aladdin'. He immediately signed on as the 'Emperor' in the show, which received rave reviews when it was performed in Toronto last year, and was "delighted" when Mr. Petty called on him last spring, this time to play 'Baron Hardup'. Shortly thereafter, Mr. Coombs departed for his annual summer trip to a home in Maine, where he was surprised to hear news over the summer of the liquidation of Eaton's. "We don't believe it's practical to have a paper and pencil test," says Bill Butcher, co -president of ETFO's Durham division. "I think teachers are tested regularly by parents and students:' And, ETFO provincial presi- dent Phyllis Benedict points out that "teachers are highly super- vised." She notes school boards have supervision and evaluation procedures in place, permanent teachers are evaluated every two to five years and the Ontario Col- lege of Teachers investigates re- ports of educators' incompetence. "There is not, however, a uni- form evaluation model for assess- ing teacher competence," she notes. "One way to focus on the issue of ensuring high profession- al standards would be to identify best practices in teacher evalua- tion and to extend uniform stan- dards across the province. Ontario's elementary and sec- ondary teachers will face compe- tency tests before the end of the school year, Premier Mike Harris said in September when he an- nounced plans to introduce the initial phase of the government's teacher -testing program in June 2000. "Implementing teacher testing would be a flagrant waste of tax- payers' money," Ms. Benedict maintains. "Providing teachers with training and resource mater- ial to support the new curriculum and giving more support to new teachers would be better invest- ments." Mr. Butcher agrees the money would be better spent on profes- sional development workshops for teachers. "It think that's absolutely es- sential to maintain current up-to- date dealings in your school," he says, adding if the Province moves ahead with plans to test teachers, then administrators, su- perintendents and school board directors should also be tested. "But there is room for improvement. "Hospitals will be able to use this research to share best practices with others as well as flagging areas for further review:" says Mr. MacKinnon. "Most important, the research establishes a base- line for future improvements and represents a major advance in accountability that will lead to improved patient care" Overall across the province, the OHA report found: - Hospitals are treating more surgical patients, treating them more quickly (with less time spent in hospitals), treating them more effectively (with fewer complications and fewer re -admissions), and there is a high level of consistency across the sys- tem. - Hospitals are admitting fewer patients for med- ical conditions monitored in the report (such as heart attack, asthma, pneumonia and stroke) and they are staying a shorter time. However, readmis- sion rates are higher, and complication rates have increased for some conditions and decreased for others. - Hospitals are reducing costs and coping with fi- nancial pressures, but their financial condition is deteriorating and their long-term debt is increas- ing. - Patients are mostly very satisfied with their inpa- tient experience in areas such as nursing care and outcome, but less satisfied with hospital support services and housekeeping. - Hospital performance in bringing about change. co-ordinating internal patient care and integrating with outside patient care services is lagging: with the majority of hospitals scoring less than half of the maximum score for most indicators in this area. In the future, the OHA plans to expand this an- nual report to include outpatient acute care services (such as emergency care), as well as rehabilitation. long-term care and mental health services. "Should I have my hip replaced?" patients often ask. Others suffering from osteoarthritis pose the same question about their knees. Many are confused about the results of these operations. How long they should put off surgery. The. long-term results. And from some letters I've received it's apparent many people have unre- alistic expectations of what they can do following hip or knee surgery. As a medical student I witnessed one of the first operations for hip re- placement performed at the Massa- chusetts General Hospital. It was considered at that time a very major The Doctor Game W. Giord-Jones, M.D. L: procedure. Now hip replacement is routine in hospitals large and small. Many peo- ple talk of having a hip or knee re- placed as if they were replacing the old tires on their car. Soon they dis- cover it's not quite that simple. Joints are wearing out because we're all living longer. Healthy joints are covered by a thick pad of slippery cartilage. This cushion protects the ends of bones like shock -absorbers on a car. But cartilage tends to wear out if you overuse a joint. Or put too much pressure on it due to obesity. Or if there's a genetic problem, or certain diseases. Cartilage can also be lost if you're a couch potato. Like any other part of See RFPLACEMF_NT page 18 Are You Tired? So many people are tired! Not getting enough sleep is the most obvious reason, but if that can be ruled out then other factors need to be investigated. From a nutritional perspec- tive, cleaning up the diet, cleansing the body of toxins and addressing vitamin and mineral deficiencies is your best bet. First of all remove refined sugar (white/brtnvn) and refined Flour (white bread, enriched Flour). This includes cake, candies, pop, ice cream and cookies. Less obvious refined sugar can be found in packaged and processed foods. Check your labels carefully. Beware of low fat food as it usually contains large amounts of sugar. White flour has few nutrients and contains no fibre. Choose 100% whole grain cereals and breads. Increase your vegetable intake to about four one-half cup servings and include one to two fruits per day providing vit- amin C, B -vitamins, anboxi- dant nutrients and fibre needed for a healthy body. Protein and essential fats are your brain food. Many peo- ple are deficient in these nutrients because of a) poor dietary choices and b) con- suming too few calories. The best protein sources include fish, chicken, turkey, lamb and red meat. Eggs and dairy are also good sources of protein. A person should eat about one gram of protein per kilogram of body weight. Essential fats (omega 3 and omega 6) are the only fats required by the body to make all of the other fats. We get these fats from foods such as fish, fresh nuts/seeds, flax oil, evening primrose oil and vegetable oils. These fats are also healthy fats for your heart. They help lower the bad cholesterol (HDLs). Many people are deficient in these fats and they are ESSENTIAL. We must consume them on a daily basis. Margarine is not a good choice as it contains trans fatty acids which disrupt the functioning of the essential fats. I would recommend that if you avoid junk foods, eat more green veg- etables, Pet optimal protein and essential fats, cleanse your body and get optimal iron, B12 and Folic Acid then you will be addressing some of the main causative factors of fatigue. Cleansing you liver and bowels of toxins is very important to NINE (a 13 -vitamin) all contribute to healthy red blood cells. A defi- ciency of these nutrients can cause extreme fatigue. Normal activity pushes nutrients into cartilage to keep it healthy. Osteoarthritic pain is caused when the rough ends of bones no longer have this protective cush- ion. Bone impinges on bone, the surfaces become rough and pa- tients complain of pain. So how long should you put up with the pain'? This is always a tough question as people have different pain thresholds. A patient contemplating hip replacement recently walked into my office with only minor diffi- culty. She was considering total hip replacement. I advised against surgery until she was ei- ther more incapacitated or in greater pain. Always ask yourself this vital question when contemplating an elective operation. "Is the physi- cal problem and pain severe enough to warrant the risk of major surgery?" If it is, the time is right. My advice as a surgeon is to delay surgery as long as it makes sense to do so. Hip and knee replacements do not last forever. They wear out in anywhere between 10 to 20 years depending on your weight, gener- al physical condition and level of activity. Agreeing too quickly to surgery may mean a second or even a third replacement opera- tion. Subsequent operations are always more difficult due to scar tissue. Like tracking a tiger, the first o sa The number of adults choosing orthodontics is on the rise. More adults are opting for healthy smiles these days. In fad, today one out of every four orthodontic patients is an adult. Regardless of a person's age, orthodontic treatment is usually a change for the better. The mechanics involved in the movement of teeth are essen- tially the same in adults as in children. Gaps between teeth, crowding, protruding front teeth and teeth in abnormal positions are problems that may be corrected in the adult by orthodontic treatment. However, because an adult's facial bones are no longer growing, certain condi- tions cannot be resolved with braces alone. Sometimes, ` surgery is required to obtain the correct result. The health of teeth, gums and supporting bone, as well as jaw relation- ships, are key factors in deter- mining the prospects of improv- ing one's appearance through -orthodontic treatment. Contrary to popular belief, wearing braces will not inter- fere with an adult lifestyle. Patients can make business pre- sentations, sing, play an instrument, dine - out and of course, kiss! The biggest` inconvenience, gen- erally, is working reg- ular visits to the orthodontist into an -adult's busy sched- ule. HEALTHY, BEAU- I TIFUL SMILES: NO GENDER GAP Self-improve- ment and interest in health and fitness are no longer primarily lust con- cerns for women. A recent sur- vey has revealed that men make up 30 percent of adult ortho- dontic patients. More and more men are recognizing the increased confidence that a good-looking smile can bring. And men also are becoming more aware of the importance of good dental health and the role of orthodontics in creating beautiful, healthy smiles that can last a lifetime. ORTHODONTICS AND ITS ROLE IN TREATING HEALTH PROBLEMS Health concerns are often the primary consideration for adult treatment. Crooked teeth and bad bites may seriously affect oral health. Orthodontic treatment might help prevent many serious potential prob- lems such as tooth decay, gum disease and eventual tooth loss. One of the most prevalent adult orthodontic problems is teeth that are too crowded. These are extremely difficult to clean, making them more prone to decay, even if good oral hygiene is practiced. Wounded animals are always more dangerous to confront. During a hip replacement the top of the thigh bone is removed and the shaft hollowed out. The surgeon then prepares the surface of the socket for the new joint. For knee replacement a por- tion of the thigh and shin bone is removed. And in each operation the lost bone is replaced by either metal or plastic parts. Not all patients with os- teoarthritis need total hip or total knee replacement surgery. For in- stance, patients can often be helped by a procedure called "arthroscopy" For this procedure the surgeon makes several small incisions and looks into the joint with optical instruments. Rough areas of bone are smoothed out and any debris in the joint is removed. Arthroscopy is more often per- formed on the knee than the hip. It is best suited for those who are not ready for a total knee re- placement. Patients can be discharged the day of surgery. And usually this approach provides pain relief for several years. Patients who submit to either a hip or knee replacement must have realistic goals. One of my patients about to have a knee replacement believed he'd be back on the ski slopes in one month! Both procedures require ex- tensive post-operative physio- therapy to strengthen muscles. There are no short-cuts to restor- ing joint function. races are i ust Conversely, spaces can also be a problem, especially if the early loss or extraction of teeth causes healthy teeth to tip into open spaces. Deterioration of the supporting bone may occur, causing teeth to become loose. Protruding front teeth, in addition to being unattractive, do not meet properly during chewing. This can cause abnor- mal wear of tooth surfaces, as well as excess stress on sup- porting bone and gum tissue. The stress on supporting tis- sues might contribute to gum disease and possibly lead to loss of teeth. And it requires a good deal of motivation to restore a joint to near normal function. How soon patients return to relatively normal walking de- . pends on several factors. The majority of those who have a hip replacement require about two to three months of re- habilitation. Knee replacement causes more post-operative pain. This surgery normally requires three months before patients can navi- gate well. In an upcoming column, I'll discuss how the use of visco-sup- plementation, using hyaluronic acid (Synvisc) can help to de- crease knee pain associated with osteoarthritis. It's a possible option for those who would prefer an injection to the scalpel. Orthodontic problems are not always confined to the mouth. More than 650 athletes rep- resenting 60 track clubs com- peted at the first indoor meet of the season. Competitors trav- elled from across Ontario, Quebec and the U.S. Paul Martin captured three gold medals in the midget boys' category. He was first in the 400 -metre race, 60 -metre hurdles and the high jump. He also placed seventh in the 60 - metre sprint. Michael Andrews finished sixth in the 400 metres and eighth in the 200 -metre sprint. Matt Sample was ninth in the 1,500 metres. Tabia Charles led the club's midget girls' contingent with a gold medal in the 60 -metre dash. She also placed third in the 200 -metre sprint.
