But now Ontario and other ju-risdictions are shifting gears; re- strictions are being lifted and a return to normal, in recent timesso difficult to imagine, seems pos- sible.Running concurrently withthe pandemic, however, Durham has been mired in another epi- demic: soaring overdoses anddeaths attributed to the use ofopioids. It is a crisis that was pre- sent prior to the onset of the pan-demic and that has continued,and worsened, as the world's at-tention focused on the virus. The result has been a strain onan already overburdened health-care system and misery for users,said Beth Brannon, senior di- rector of mental health and ad-dictions at Lakeridge Health. "This is a crisis that hasbeen evolving over several years. When you look at thepublic health numbers,you look at the paramedic calls, DRPS calls, you canlook at the number ofdeaths, we're seeing all ofthose climb. You can see them climb across theprovince and the country,actually," Brannon said. "The most staggeringfor us is the (emergency de-partment) visits," she add-ed. "We've seen a 217 per cent increase since 2017,which is really quite dra-matic."What was already an ep- idemic of addiction hasbeen worsened by a pan-demic that has disrupted services and isolated userswho rely on an often toxicand sometimes deadlydrug supply, according to Dr. Sajida Afridi, divisionhead of addiction medicineat Lakeridge Health. "The COVID-19 pandem-ic has clearly exacerbatedall the numbers and thereare multiple factors that are responsible," she said.The drug supply is in-creasingly tainted with substances that includefentanyl, carfentanil andbenzodiazepines — fenta-nyl alone has been deter- mined to have contributedto 87 per cent of opioiddeaths recorded during the pandemic, Dr. Afridi said."So that was one signif-icant driver. The secondwas public health mea- sures, which includedphysical distancing. Thatcaused a huge reduction inthe availability (of) and ac- cess to services. Therewere reduced pharmacyhours for our patients who were going for methadoneand suboxone dosing everyday," Dr. Afridi said. "Thenthere were reduced hours for harm-reduction servic-es. And many people whowere attending in-person counselling and supportgroups were unable to at-tend those — and those were their social network."Statistics released bythe Region of DurhamParamedic Services indi- cate the service respondedto 998 calls for suspectedopioid overdoses in 2021, an increase over the total of725 such calls in 2020.In 2019, the paramedicservice responded to 591 calls, according to the sta-tistics.Confirmed opioid deaths also appeared to betrending upward in 2021, al-though those statistics re-mained incomplete at the time the end of the year re-port was released. BetweenJanuary and July of 2021,some 73 deaths had been confirmed, according tothe report. There were a total of 94 opioid-related deaths in allof 2020, according to thestatistics. That's five timesthe number of fatalities confirmed in 2013."The number of deathsrelated to opioid poisoning in Durham Region resi-dents has been increasingover the past eight years,"the report states. In 2020, two-thirds of thecalls for suspected ODs —66 per cent — originated in Oshawa, a trend that hasbeen apparent for sometime, according to the data. Newer statistics indi- cate that from the begin-ning of 2022 until the end ofFebruary, Durham para-medics responded to 80overdose calls, a reductionfrom the 112 recorded dur-ing the same period in 2021. The issue is one of acuteimportance for OshawaMayor Dan Carter, who ad-vocates for a multipronged approach that includes de-livering services to thosecaught in the addiction trap while bringing theforce of the law to bear ondrug dealers who prey on avulnerable cohort. "The one area that frus-trates me more than any-thing is the victimization. We have to understand thatmany of those who are un-sheltered or living on our streets have severe, com-plex mental health issues.And on top of that, many of them are also dealing withan addiction issue. Andthey're being preyed upon,"said Carter. "We're trying to figureout how do we get into con-tact with these individuals. How do we get them intothe right stream, so we cansupport them so that theywon't be victimized." Carter recently made apresentation to the Dur-ham Regional Police Ser- vices Board, seeking policesupport to fight drug traf-ficking downtown. Dealersneed to be identified and prosecuted, he said."They have to under-stand that selling poison topeople who live on ourstreets, for profit, is againstthe law. And I want thoseindividuals held account- able," he said. "But the oth-er side of it is, those individ-uals who are seeking outthose drugs, we've got to help those individuals."Despite the gloomy sta-tistics, Lakeridge's Bran- non takes heart in what shesees as a genuine commit-ment by relevant commu-nity agencies to tackle the opioid issue."This is an incrediblycomplex program that no one agency, no one service,no one industry will solvealone," she said. "I am in- credibly proud of how thisregion has come togetherto try and respond to this. You've got health servicesworking with police, work-ing with housing, workingwith public health ... to say, how can we wrap aroundthese people and get themthe care not only that they need, but that they deserve.I think there's been a hugecoalition that's come to-gether." On the front lines in theemergency rooms of Lake-ridge hospitals, in addition to providing immediatetreatment for OD patients,efforts are made to presentthem with the information they need to address theiraddiction and other mentalhealth issues, Dr. Afridisaid."We are seizing these op-portunities to connect peo-ple with social services," she said. "We have adoptednovel approaches. We havegone virtual as well. We aremeeting the clients where they are. Not only are weoffering in-person appoint-ments, we are offering vir- tual consultation. "All these flexible ap-proaches have really ex-panded access and made it more equitable for our cli-entele." SAFE INJECTION SITES ACONTROVERSIAL MEASURE As Durham Region deals with an ongoingopioid crisis, safe injectionsites have been among thesolutions considered. The issue was debatedin Oshawa, where in 2019city council voted to reject a proposal to create a sitewhere people could usedrugs in a supervised set-ting. In a recent interview,Oshawa Mayor Dan Carterstood by that decision. Hesaid more focus needs to beplaced on a safe supply —trying to ensure that toxicdrugs are not circulating in the community."I think when people goimmediately to safe injec-tion sites, they forget about all of the other tentaclesthat go with that," he said. "I'm not in support of safe injections sites asthey're being operated atthis time; I think we have tohave a bigger conversation about the supply issue,"Carter added. "I havetalked to law enforcement in Toronto in regards tosafe injections sites and theproblem is, there's like thistwo-block radius of a no go zone. And the drug dealers,all they do is operate in thattwo-block area. To me, that just doesn't make anysense."Offering an alternativeview is Dr. Sajida Afridi, di- vision head of addictionmedicine at LakeridgeHealth."Supervised injection sites are evidence-based in-tervention and they arepart of the harm reduction principle and approach,"she said. "These sites aresafer and there is no associ-ation with increased rates of crime, which I think wasaconcern for many com-munities when they looked at supervised injectionsites. These sites really savelives and they have (been)shown as cost-effective." "If there is one evidence-based solution available, Ithink we should proceed in that direction," Dr. Afridisaid. "It's part of the harmreduction principle." In a matter of months,Tenneia Francis wentfrom being uncertain about university as apathway to applying —and being accepted — to several universities.She will be heading toYork University in thefall to study psychology. "I'm blown away be-cause university wasnever something we had discussed. I'm in awe, I'mproud of her," her momsays. The Ministry of Edu- cation launched theGraduation Coach Pro-gram for Black Students as a pilot in 2019.Since then, about1,000 students in Ontariohave benefited from it. The DDSB has re-ceived four years offunding for the program; it is being offered at PineRidge Secondary Schoolin Pickering this year,and is expected to ex- pand to other highschools in the comingyears.Why a graduation coach for Black stu-dents?An analysis of data from the DDSB's firststudent census conduct-ed in 2019 looked at aca-demic outcomes for sev- eral identities, includinga comparison of white,Black and South Asian students. Ninety-five per centof DDSB students whoidentify as South Asian graduated from highschool in four years com-pared with 89 per cent of white students and 85per cent of Black stu-dents.The analysis also showed that Black stu-dents had higher pro-portions of suspensions compared to their re-presentation in the stu-dent census.The data is not unique to the DDSB.School boards acrossOntario report similar findings. Her guiding light throughout her life was her holy bible and the love for her family. She was a joyous and spiritual woman whose smile and laughter will be missed dearly.
